Open Monday to Friday 8:30 AM to 4:30 PM
• Referrals are required for Mental Health Services unless an individual receives primary care services with Acacia Network
• Psycho-social and additional forms (ask the organization to email them) are required
• Intake is available Monday-Friday, 9:00 AM - 12:00 PM, on a first-come, first-served basisMust complete an intake
