Voluntary 6-12 month contract for housed individual (Homebase visits only, requires stable housing):
• Referrals to therapy
• Rental arrears and HRA
• OPWDD, Special education support
Open Monday to Friday 9 AM to 5 PM
General population can get preventive services by calling (212) 676-7667, no walk-ins are allowed.
Only serves people who are clients of the organization
