  • Only NYS Medicaid recipients are admitted

  Requires a photo ID

  Requires a birth certificate

  Requires proof of health insurance

  Requires a referral package: psychosocial assessment, basic lab report, health insurance card, and psychiatric evaluation

  Age requirement: 18+
