Services are for adults only, except primary care and nutrition (all ages, with referral):

• Health Insurance Enrollment
• Nutrition
• GYN
• Pediatrics
• Podiatry
• Primary care

• Open Monday 8 AM to 5 PM; Tuesday to Wednesday, Friday 9 AM to 5 PM; Thursday 10 AM to 5 PM

• 
  • Patients under 18 need a legal guardian present
  • MetroCards are provided to Medicaid recipients
  • All services are available by appointment, but you may walk in to sign up
  • Staff can assist you with transferring your Primary Care provider
  • More services are available at the Manhattan location

• Only serves people who are clients of the organization

  Requires Photo ID

  Requires Proof of Health Insurance or Income to be placed on a Sliding Fee Scale.
