EAC Network (Empower, Assist, and Care)

Services include:
• TASC
 — Substance Use Diversion: Focused on substance use, mental health, and diversion for individuals facing criminal charges.
 — Mental Health Diversion: A program providing mental health support as an alternative to incarceration.
• Enhanced Employee Initiative for Employment Opportunities: Initiative aimed at increasing employment access for individuals facing employment barriers.
• Health Home Case Management: Comprehensive support services for individuals with chronic conditions, ensuring coordination of care.
• CRAN (Comprehensive Recovery and Assistance Network):
 — Long-Term Services: Ongoing mental health support for individuals requiring extended care.
 — Short-Term Services: Immediate mental health support for individuals in need of urgent care.
 — Referral Services: For mental health and substance use support through case management.
 — Case Management for Individuals Recently Incarcerated: Support services for people released from Rikers Island within the last two years, aged 18+.

• Open Monday to Friday 9 AM to 5 PM

• People interested in CRAN can walk in; people interested in TASC are usually referred through the court system.
