  Alternatively, you may pick it up and drop it off in person during open hours, and attach the psychosocial attestation or attestation of a mental illness by a clinician.
  
  After submitting, schedule a tour between 10:00 AM - 12:00 PM and 2:00 PM - 4:00 PM, Monday through Friday, by calling the main number and dialing extension 0. The best time for a walk-in is 11:30 AM, have lunch, then complete the intake.
  
  Any NYS resident is eligible if they possess any SMI diagnosis.
