Services for residents of community districts 1, 2, 3, 5, and 6:
• Case management: enrollment in benefits
• Meals on Wheels and home health aids for homebound or disabled people who have Medicaid
• Affordable housing (waitlist), home repairs, home sharing, guardianship
• Transportation

• Open Monday to Friday 9 AM to 5 PM

• Age requirement: 60+
