• Psychiatric Assessment
• Gateway-to-Wellness Care Coordination:
 — Appointment Scheduling
 — Education Referrals
 — Housing Referrals and Assessment
 — Integration of Medical and Behavioral Care
 — Peer and Family Counseling/Support

• Open Monday to Friday 8 AM to 8 PM; Saturday to Sunday 8 AM to 6 PM

• Walk in or call to schedule an assessment.

• Only serves people who are clients of the organization

  Must complete an intake

  Requires proof of health insurance

  Age requirement: 4+
