• Open Monday to Friday 9 AM to 5 PM

• 
  • Self-pay is available to people ineligible for insurance
  • Walk-ins are welcome
  • If you are a Medicaid or Medicare recipient, see if you qualify for a Round-Trip MetroCard upon your visit
  • Age requirement: 18+
  • People of all genders are welcome

• Requires health Insurance or proof of income (for a sliding scale fee)

  Requires a Referral Package: Psychosocial Assessment, Basic Lab Report, Health Insurance Card, and Psychiatric Evaluation

  Requires a photo ID
