• Open Monday to Friday 9 AM to 7 PM; Saturday 9 AM to 2 PM

• Call to schedule an intake appointment or to forward a referral

• Only serves people who are clients of the organization

  Must complete an intake

  Requires Letter from HRA confirming you can receive benefits in New York State

  Requires Insurance or proof of income for a fee scale

  Requires Photo ID

  Requires Referral Package: Psychosocial Assessment, Basic Lab Report, Health Insurance Card, and Psychiatric Evaluation

  Age requirement: 12+
