• Open Monday to Friday 8:30 AM to 8 PM

  • Intake Hours are Mon-Fri, 8:30 AM- 4 PM
  • Most major insurance plans are accepted, as well as Medicaid and Medicare
  • Sliding fee scales are available to people ineligible for insurance
  • Referral packages can be forwarded to the intake department

• Must have history of Substance Use and complete an intake

  Requires referral package: Psychosocial, Recent lab results, Psychiatric evaluation if necessary

  Requires proof of health insurance or income for sliding fee scale

  Requires a photo ID

  Age requirement: 18+
