• Open Monday to Friday 8:30 AM to 5 PM

  • You can schedule an appointment with the evaluation and referral unit to complete an assessment
  • Following an assessment, patients are referred to the behavioral health or substance use program that meets their needs

• Only serves people who are clients of the organization

  Requires Active health insurance
