  • The facility is open Mon-Fri, and every 2nd Saturday of the month
  • If you are a Medicaid recipient, see if you qualify for a Round-Trip MetroCard upon your visit. Straight Medicare incurs copays. HealthPlus, Empire, and Blue Cross/ Blue Shield are not accepted.

• Only serves people who are clients of the organization

  Requires active Health Insurance

  Requires referral from social service provider if necessary
