• — Fee-based mild-moderate dementia treatment "Social Adult Day Care" for everyone (all ages and counties) 9/9:30 AM - 2 PM
• — Free services for members from community districts 8, 11, and 13:
• Case management:
• * You can request home-delivered meals $2 voluntary donation
• Entitlements (Tax rebates, Medicaid HEAP). If you are a non-citizen, please see if you might qualify for this service.
• Transportation to medical appointments within Eastern Queens/Nassau County ($10 voluntary donation), Transportation to Stop&Shop and other stores around
• Breakfast is at 9, Lunch is at 11, $2.50 voluntary donation
• Exercises, seminars, and other activities throughout the day

• Open Monday to Friday 9 AM to 3 PM

• Feel free to walk in to take a tour and sign up.

• Age requirement: 60+
