• Open Monday to Wednesday 9 AM to 8 PM; Thursday to Friday 9 AM to 5 PM

  • You can submit a new patient form or make an appointment for an existing patient or you can call to enroll/make an appointment
  • Most health insurance types are accepted
  • Sliding fee scales are available to uninsured people
  • If you are a Medicaid or Medicare recipient, see if you qualify for a Round-Trip MetroCard upon your visit.
