Services include:
• Pediatrics on Saturdays. All underage patients are served. The rest of the services are available on weekdays
• Primary Care, Podiatry, Infectious Diseases, and MAT. Age requirement: 18+
• Dentistry. Age requirement: 2+
• Mental Health and Psychiatric Evaluations. Age requirement: 18+

• Open Monday, Wednesday to Friday 8 AM to 6 PM; Tuesday 8 AM to 7 PM; Saturday 9 AM to 2 PM

• 
  • You can submit a new patient form, make an appointment for an existing patient or you can call to enroll/make an appointment
  • Most health insurance types are accepted
  • A sliding fee scale is offered to people without insurance
  • If you are a Medicaid or Medicare recipient, see if you qualify for a Round-Trip MetroCard upon your visit.

• Requires health insurance or proof of income and identity (for a sliding scale fee)
