• Pediatrics. Age requirement: under 18
• Primary Care. Age requirement: 18+
• Urgent Care. Age requirement: 3 months+
• Dentistry. Age requirement: 2+

• Open Monday to Thursday 8 AM to 6 PM; Friday 8 AM to 5 PM

• 
  • You can submit a new patient form, make an appointment for an existing patient or you can call to enroll/make an appointment
  • Most health insurance types are accepted
  • A sliding fee scale is offered to people without insurance

• People of all ages are welcome
