• Open Monday to Friday 9 AM to 5 PM

  • No walk-ins allowed
  • Call during office hours to schedule an intake appointment
  • All health insurance plans are accepted except for Medicare
  • A sliding scale fee is available for clients without coverage and is available based on family size and income

• Must have a history of Substance Use Disorder or a Court Mandate

  Requires a photo ID

  Requires Proof of Health Insurance or Income to be placed on a sliding fee scale

  Age requirement: 18+
