• Vaccines
• Health Insurance Enrollment

• Open Monday to Friday 7 AM to 5 PM; Saturday 7 AM to 1 PM

• 
  • You need to see Primary Care or Pediatrics to receive Specialty Services
  • You do not have to be from New York City/State to receive a sliding fee scale or services, but your insurance might not cover the services if you are out of state

• Requires a legal guardian present with birth certificate for the patient (if under 18)

  Requires proof of address and health insurance/proof of income (for a sliding fee scale)

  People of all ages are welcome
