• Open Monday to Tuesday, Friday 9 AM to 5 PM; Wednesday 8 AM to 6 PM; Thursday 8 AM to 5 PM; Saturday 8 AM to 4 PM

  • Please provide the required documents to receive services
  • You must be a patient of primary care or pediatrics to receive any specialty services
  • MetroCards are not provided to Medicaid recipients
  • You do not have to be from New York State to receive fee scale or services

• Requires legal guardian present with birth certificate for the patient (if under 18)

  Requires Proof of Health Insurance or Income to be placed on a Sliding Fee Scale.
