  • You must be a patient of primary care or pediatrics to receive any specialty services
  • MetroCards are not provided to Medicaid recipients
  • You do not have to be from New York State to receive fee scale or services, most health insurance types are accepted, check with your insurer if you are from out-of-state if you qualify for coverage

• Requires a proof of health insurance, a photo ID, and a proof of income (if a sliding scale is needed)

  Requires a legal guardian present with a birth certificate for an underage patient

  Age requirement: 13-18 (until your 19th birthday)
