• Open Monday to Friday 8:30 AM to 9 PM

• 
  • Please call or walk in to enroll. Medicare is not accepted. If you are a Medicaid recipient, see if you qualify for a Round-Trip MetroCard upon your visit.
  • For intake, walk in on Mondays at 2:30 PM, Thursdays and Fridays at 5:00 PM, or Saturdays at 11:30 AM.

  Requires health Insurance or proof of income (for a sliding scale fee)
