• (Including for OPWDD eligibility,
• guardianship evaluation, and autism
• assessment)
• Psychosocial Evaluation
• Psychotherapy
• Sexual Consent Determination
• Speech Therapy

• Open Monday to Friday 8 AM to 5 PM

• You can call to schedule an appointment.

• Only serves people who are clients of the organization

  Must complete an assessment and have an intellectual and or developmental disability

  Requires referrals from service providers

  Requires Birth certificates

  Age requirement: 6+
